Member and Visitor Vaccination Declaration Now required as part of self-screening process

The following vaccination declaration is now part of the YMCA's self-screening process for access to Y programs and facilities.

COVID-19 Vaccination Declaration

Effective: September 1, 2021

The YMCA of Eastern Ontario strives to protect the health and well-being of everyone who attends its facilities and programs Given the risks associated with COVID-19, this policy requires all members and visitors to the YMCA of Eastern Ontario to be fully vaccinated unless medically exempt or ineligible. COVID-19 vaccination refers to vaccines approved by Health Canada.

To access the Y facilities or programs, you must meet and maintain one of the following categories of vaccination status. Please select one of the following:

  1. I am fully vaccinated for COVID-19 and am able, if requested, to show proof of vaccination.
  2. I have received my first COVID-19 vaccination and will take one Covid test per week until I receive my second vaccination. I may be asked to show proof of negative test results.
  3. I am unable to be vaccinated for COVID-19 for medical reasons or am ineligible to be vaccinated, and can supply, if requested, a doctor’s note in support of this. I must take one Covid test per week further notice. I may be asked to show proof of negative test results.
  4. I am not currently vaccinated for COVID-19 and understand that, until further notice, I must take one Covid test per week until I am fully vaccinated. I may be asked to show proof of negative test results.

By buying a day pass, attending a program or swiping my membership card, I declare that I have answered “no” to all health questions and have passed the health check, and that I am fully vaccinated, if eligible, or have taken a recent COVID-19 test and received a negative result. I understand that may be asked to show proof of vaccination or of negative test results.

This policy may be updated subject to federal, provincial and/or public health decisions.
